Medicare Mental Health Coverage: What's Included in 2026

Medicare covers a full range of mental health care: outpatient therapy and psychiatry under Part B (you pay 20% after the deductible; often $0 with a supplement), inpatient psychiatric care under Part A, and mental health medications under Part D. Since 2025, telehealth therapy at home is a permanent benefit, and yearly depression screening is free.

What outpatient mental health services does Medicare Part B cover?

Mental health care is one of Medicare's most underused benefits. Roughly one in six older adults lives with clinically significant depressive symptoms, yet the large majority never receive treatment, a study of nearly 3,000 older adults found that about 78% of those with depression were not being treated (Nguyen et al., Innovation in Aging/PMC). Often, the barrier isn't coverage. It's that nobody explained the coverage.

Part B is where most mental health care lives. It covers:

  • Individual psychotherapy with a psychiatrist, psychologist, LCSW, nurse practitioner, marriage and family therapist, or mental health counselor
  • Group psychotherapy
  • Family counseling when the main purpose is treating your condition
  • Psychiatric diagnostic evaluation and psychological testing
  • Medication management
  • Annual depression screening, free, with no coinsurance or deductible, in a primary care setting
  • The "Welcome to Medicare" visit and Annual Wellness Visits, which include a review of your risk for depression
  • Partial hospitalization and intensive outpatient programs for more structured care

What do you pay?

After you meet the annual Part B deductible, Medicare pays 80% of the approved amount for outpatient mental health services; you're responsible for 20%, unless you have supplemental coverage:

  • Medigap (Medicare Supplement): most plans pay the 20% coinsurance, bringing therapy to $0 out of pocket
  • Medicaid (dual eligibles): typically $0
  • Medicare Advantage: copays vary by plan; mental health coverage must be at least as generous as Original Medicare
  • Retiree/secondary insurance: often covers the balance

Free preventive services: the annual depression screening and wellness visits cost nothing.

What does Medicare Part A cover for mental health?

Part A covers inpatient psychiatric care, in a general hospital or a psychiatric hospital. Note one unique rule: care in a freestanding psychiatric hospital is subject to a 190-day lifetime limit (this limit does not apply to psychiatric units within general hospitals).

What does Part D cover?

Prescription drug plans cover mental health medications, including antidepressants and anti-anxiety medications. Part D plans are required to cover substantially all antidepressants and antipsychotics as protected drug classes.

Recent changes that matter for seniors (2024-2026)

  1. More therapists accept Medicare. As of January 1, 2024, marriage and family therapists and mental health counselors can enroll in and bill Medicare, adding hundreds of thousands of potential providers (Telehealth.HHS.gov).
  2. Teletherapy at home is permanent for behavioral health, with audio-only allowed, and no in-person visit required through at least December 31, 2027 (CMS Telehealth FAQ, Feb 2026).
  3. Intensive outpatient program (IOP) coverage began in 2024, filling the gap between weekly therapy and hospitalization.

Depression in later life is not a normal part of aging, and untreated, it worsens outcomes for heart disease, diabetes, and cognitive health, and increases mortality (StatPearls/NCBI). The good news: late-life depression responds well to treatment, psychotherapy approaches like CBT and problem-solving therapy have strong evidence in older adults, and structured, measurement-based care helps ensure treatment is actually working.
